Parent's Diary

For the second part of this study we would like you to complete brief anonymous electronic questionnaire's at four (first session, end of the first month of sessions, end of the second month of sessions, and end of the study) of  your child's biofeedback sessions as part of your diary entries. This will take you approximately 15-20 minutes to complete.

Questions will look to determine any changes seen from your child following their participation in the biofeedback sessions, as well as your own personal experiences of stress and coping with your child's behaviour.

Your responses will be collected with other volunteer participant responses and quantitatively analysed as an aggregated whole, ensuring that no individual responses are identified. Your results will be sent to Dr. Andrew Campbell, the head supervisor for this research project. The data collected from all questions will be stored on a secure 128-bit encrypted server, used to safe guard participant's privacy and confidentiality.

Before beginning, you will be asked to create a personal six alpha-numeric code. Please remember this code as you will be required to use it for every entry. Store or record it somewhere safe in the event you wish to withdraw from the study, without it we are unable to locate your responses for deletion. To withdraw from the study, contact Dr. Andrew Campbell and quote your personal code, and your responses will be deleted accordingly.

By pressing the "SUBMIT" button at the end of the electronic diary, you are consenting that your diary responses may be used as part of the study of "AD/HD Management through Biofeedback".

IMPORTANT: Please complete a diary for EACH of your children participating in the biofeedback sessions. We need completed Diary entries for all children, with and without AD/HD. Please try and complete the questionnaires in the week they are required to ensure behaviours are reported in the Diaries of that time frame.
